Declaring that you have tested positive for COVID-19 will help limit the spread of the virus and protect the health and safety of the community. Completing a self-declaration form is not mandatory, but is strongly recommended.

The form will be sent to the COVID-19 Coordination Unit at Université Laval, and you will get an acknowledgment of receipt. The information you provide could be forwarded to a public health authority for an epidemiologic investigation.

The person responsible within your faculty or department (program director, research director, internship supervisor, person responsible of the academic unit or professor) will be notified of the duration of your quarantine and your isolation end date, according to your declaration. If you have any assessments scheduled during this time, you must make arrangements with your program director and professors.

If you plan to stay at the University’s residences following your quarantine, the Residence Office will also be notified of your isolation end date in accordance with your declaration.

The information provided will be securely logged and destroyed no later than 30 days after the end of the quarantine.
